Home Health Registered Nurse

Day Shift: 5x8-Hour (08:00 - 17:00)

This is a 13-week contract position.

The Home Health Registered Nurse will provide skilled home care visits to patients with diverse medical needs, including post-surgical recovery and chronic conditions. The role involves direct patient care and coordination with interdisciplinary teams to ensure optimal health outcomes.

Responsibilities:

• Conduct comprehensive patient assessments and develop care plans.
• Provide nursing care for patients with conditions such as cardiac, orthopedic, respiratory, and neurological issues.
• Administer medications and treatments as prescribed.
• Perform OASIS assessments and document care using BayCare Forms and scheduling tools.
• Manage up to 8 patient visits per day, ensuring high standards of care.
• Educate patients and families on health management and discharge planning.
• Maintain compliance with health regulations and safety protocols.

Required Experience / Certifications / Licensure:

• Minimum of 1 year of home health experience.
• Valid RN license in Florida or Compact state.
• BLS certification required.
• OASIS training preferred.
• Must possess a valid driver's license and have access to a car for home visits.
• COVID-19 vaccination required or approved exemption on file.
• Flu shot required or declination with mandatory mask wearing while at work.
