Overview This individual is a clinically competent Cardio‑Vascular Advance Practice Clinician (APC) who will directly report to the Director of APCs with a matrix reporting structure to the Executive Director of the Cardiac and Vascular Service Line. S/he must be able to effectively deliver care through demonstrated expertise to the Cardio‑Vascular patient. The principal duties and responsibilities include but are not limited to performing pre‑procedure assessments, histories and physicals, education and counseling of patients on cardiac and vascular diseases and treatments, tentative diagnosis, ordering and preliminary interpretation of lab studies, post‑procedure management of patients, and discharge planning. Responsibilities Provides timely and thorough pre‑procedural workup on all Cardio‑Vascular patients to include orders, history and physical, review of consent and lab tests. Provides post‑procedural orders, discharge planning and education on all Cardio‑Vascular patients. Provides direct patient care and evaluates patient outcomes. Participates in the evaluation of closure devices, sheath pulls and arteriotomy sites. Performs orders and interprets results of diagnostic and lab tests on all pre‑ and post‑cardio‑vascular patients. Provides training and education to the CCL staff, ED and inpatient units, staff and community as needed. Actively participates in mock codes and cardiac arrests on cardiac units. Actively participates in review of PI data and all PI activities associated with the cardio‑vascular patient. Assessment & Management Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Orders and collects data using appropriate evidence‑based assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Diagnosis Collaborates with the interdisciplinary health‑care team in making diagnoses of acute and complex chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority. Diagnoses complications and orders appropriate interventions. Formulates Plan of Care Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health‑care needs. Utilizes evidence‑based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum. Implements and modifies plan of care. Prescribes diagnostic strategies and therapeutic interventions. May perform advanced procedures consistent with privileges and competency validation. Communication and Collaboration Maintains ongoing communication and collaboration with the interdisciplinary health‑care team. Facilitates and communicates with patient, family and staff to promote continuity of care across the inpatient and outpatient continuum of care. Documentation Documents and dictates key components of patient progress via daily progress notes, transfer and discharge summaries, and clinic notes where applicable. Documentation is timely, meets acute care compliance standards and captures patient acuity. Professional Practice Demonstrates professional practice behaviors including preceptor/mentoring, education and instruction of graduate students. Seeks opportunities for active engagement in research and analysis of evidence‑based practice. Actively participates in Advance Practice Grand Rounds and specialty‑specific meetings. Maintains CEUs and membership in a professional organization. Qualifications Minimum 3 years of experience in clinical nursing. Minimum 2 years of experience functioning as an APC in the critical care cardio‑vascular setting. Preferred settings include but are not limited to the Cardiac Catheterization Lab, non‑invasive stress testing and vascular procedural area. Must have a strong clinical background, strong communication and motivational skills. Must have knowledge of pharmacology principles, dysrhythmia, ability to assess clinical situations and initiate appropriate treatment, interpret laboratory data, respond to emergency situations, and coordinate work schedules to achieve maximum productivity and efficiency. Competencies Skills Experience in hospital or acute care setting Clear communication skills both written and verbal Knowledge of basic computer programs Ability to keep confidential information regarding patients and team members Ability to withstand crisis situations Skills to provide customer service to patients, team members and visitors Knowledge and experience with electronic health records Credentials NP – Nurse Practitioner ACLS – Advanced Cardiac Life Support BLS – Basic Life Saving certification Education Master of Science in Nursing #J-18808-Ljbffr
