JOB SUMMARY:

This position is a liaison for Kids Central Case Management and community agencies providing medical, dental, and mental health services for the children and parents served through the dependency system. To perform this job successfully, an individual must be able to perform each essential duty listed below satisfactorily. ESSENTIAL FUNCTIONS:  

• Attend client staffings, to include but not limited to case transfer staffings on medical neglect or medically complex, Baker Act, multi-disciplinary, permanency, and Children’s Medical Services (CMS).
• Attend medication management appointments and assist with obtaining medical reports and appropriate forms for children in care.
• Assist case managers to ensure all children in out-of-home care who are on psychotropic medications have all required documentation (psychotropic treatment plans, etc.) in the case file and are filed with Children's Legal Services (CLS).
• Conduct ongoing reviews with case managers and supervisors for children on psychotropic medication to ensure proper follow-up and documentation has been completed and answer questions as needed about the medication and/or diagnosis.
• Assist case managers with updating psychotropic, medical, immunization, and educational Florida Safe Families Network (FSFN) entries.
• Contact caregivers of children who are on psychotropic medications to ensure caregiver is educated on how to administer medication and answer any questions.
• Complete monthly reports.
• Attend meetings with Sunshine Health and other health care providers.
• Complete Health Readiness Assessment (HRA) forms for children in care. 
• Make sure all children in out-of-home care have routine medical and dental care. Assist case managers with scheduling medical and dental appointments.
• Assist with medication administration for children in care in the office as needed.
• Assist the case manager and or caregivers with any medical questions they may have.
• Other duties as assigned.

  QUALIFICATIONS:

  License/Certification/Special Requirements:

• Licensed Practical Nursing (LPN)

Education/Experience:

• Graduate from a state-accredited school of Practical Nursing.
• Must have coursework training or experience that provides a working knowledge of mental health diagnosis, substance abuse issues, and related treatment recommendations.

  KNOWLEDGE, SKILLS & ABILITIES:

• Ability to work in a highly visible position and use effective interpersonal communication skills to respond to children and their families, and legal entities (i.e., judges, law enforcement, CLS etc.). 
• Ability to read, analyze, and interpret professional journals, procedures, or governmental regulations.
• Ability to write concise reports within 24 hours of the event or as required by the program. 
• Must be able to define problems, collect data, establish facts, and draw valid conclusions.
• Ability to interpret an extensive variety of instructions and deal with abstract and concrete variables. 
• High level of organizational skills.

• Good visual acuity and ability to communicate.
• Ability to lift, lower, push, pull, and retrieve objects weighing a minimum of 20 pounds. Reasonable assistance may be requested when lifting, pushing, and/or pulling that exceeds the minimum requirements.
• Ability to withstand prolonged standing and walking. Also able to sit, use hands to finger, handle, or feel; reach, stoop, bend, and/or kneel during an 8-12 hour day.
