This is an in‑person RN position at both Geisinger Wyoming Valley and Geisinger South Wilkes Barre, providing on‑site support to inpatient care teams. Job Duties & Responsibilities At the heart of inpatient care coordination, ensuring patients receive the right care, at the right time, in the right place. Collaborate closely with interdisciplinary teams while making a meaningful difference in outcomes that matter to patients and families. Provide on‑site support to inpatient care teams in a fast‑paced, mission‑driven environment. Typical work week Monday through Friday, 8:00AM‑4:30PM; occasional coverage of approximately every third weekend. Assess, plan, implement, coordinate, monitor, and evaluate all options and services to optimize patient or member health status. Manage utilization and practice metrics to refine care delivery models for maximum clinical, quality, and fiscal outcomes. Integrate evidence‑based clinical guidelines, preventive protocols, and other metrics into patient‑centric treatment plans. Develop systems of care that monitor progress and promote early intervention in acute care situations. Assist with design, implementation, and evaluation of the advanced patient‑centered care model. Qualifications At least 2 years of prior RN experience in a hospital setting. Prior case‑management experience preferred (not required). Must be available to work on‑site at Geisinger Wyoming Valley (GWV) or Geisinger South Wilkes Barre (GSWB). This role is exempt and salaried. Education Graduate from a Specialty Training Program in Nursing (Required). Experience Minimum of 2 years in nursing (Required). Certification & License Licensed Registered Nurse in the State of Pennsylvania – RN_State of Pennsylvania.
