Required: Residency training in Family Medicine, Emergency Medicine, Internal Medicine, or Physical Medicine & Rehabilitation. Required: Fellowship in Primary Care Sports Medicine (1 year) from an accredited program.State Medical License to practice in Pennsylvania. Board Certified or Board Eligible in primary specialty (e.g., Family Medicine, Internal Medicine, PM&R) with a Certificate of Added Qualification (CAQ) in Sports Medicine.
