Job Description
Registered Nurse - Care Manager

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs. The overall goal is to enhance the quality of patient management and satisfaction while promoting continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning. The Care Manager must be a highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies.

Essential Duties

• Identify and prioritize patient cases by reviewing work lists and recognizing new admissions.
• Conduct and document assessments and plans of care in the electronic health record system per departmental guidelines.
• Participate in daily care management meetings, adhering to established protocols.
• Consult with the social worker as per established criteria.
• Communicate priorities with the Care Management Assistant as necessary.
• Attend and actively participate in multidisciplinary team meetings to provide and receive information on patient progress.
• Alert care teams to concerns impacting anticipated discharges and assist with discharge readiness.
• Modify discharge plans based on information shared in meetings.
• Assist in identifying the expected discharge date for patients.
• Engage in follow-up actions from care management meetings as appropriate.
• Attend weekly complex care meetings and present on patients, collaborating to identify trends and resolve issues.
• Monitor high-risk cases and escalate them when necessary.
• Discuss barriers to discharge and psychosocial concerns with multidisciplinary team members.
• Coordinate family meetings as necessary to support patient care progression.
• Provide education regarding community resources and appropriate support groups to patients and families.
• Facilitate referrals to community resources and post-acute providers as necessary.
• Communicate medical milestones for transition with patients and families.
• Monitor observation patients to ensure appropriate care progression.
• Assess discharge plans and identify post-discharge needs, communicating this information to relevant parties.
• Verify patients' understanding and agreement on discharge plans.
• Refer administrative tasks to the Care Management Assistant.
• Consult with a social worker and/or utilization manager as per departmental protocols.

Required Skills & Experience

• Two (2) years of healthcare experience as a Registered Nurse.
• Strong assessment and critical thinking skills.

Preferred Education

• Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) from an accredited school of nursing.

Required Certifications & Licensure

• Licensed to practice as a Registered Nurse in the state of North Carolina.

Schedule

This role involves coordinating care for acutely ill patients with complex medical conditions and discharge planning needs. Candidates with recent experience as a case manager, care manager, or discharge planner within an acute care hospital environment are preferred. Shift hours are Monday through Friday, 8:30 AM to 5:00 PM.
