ABOUT THE JOB
As apart of the Complex Care team you will provide case management services for adults in jail diversion, individuals involved in Aid & Assist, Community Restoration, at Oregon State Hospital, under Civil Commitment Investigation, as well as individuals in Acute Care settings. Your skills in problem solving, time management, communication, crisis support, and advocacy will be instrumental in your success within this role as you partner with jail staff, team members, internal PCBH partner programs and other community agencies to facilitate an individual's success in their transition to the community.
As a member of our team, you will work with adults who require assistance in transitional planning, care coordination, navigating complex systems and case management. Duties include identifying and assisting individuals in obtaining supports and entitlements including Oregon Health Plan, housing, physical health, employment, substance abuse and mental health services. Other duties associated with this position may include care coordination with community partners, service referrals, completion of screenings and assisting the individual in developing a transition plan to support engagement and retention in services, as well as a successful reentry into the community. This position will require you to accurately utilize skills training, complete administrative tasks related to transitions and reporting, ability to utilize Excel Spreadsheets to track client data, coordinate and track referrals to residential providers, and coordinate with community partners in a professional manner to facilitate the continuum of care.

The client has a historical diagnosis of Bipolar Disorder, past psychiatric hospitalizations, and a history of substance use, but they have been sober for the past 7 years. The client has done well in structured environments but has struggled in less structured settings. A residential provider requests background information to assess placement. - High School Diploma or equivalent and three years of responsible and relevant professional experience; OR any equivalent combination of training, education, and experience that provides the required skills and knowledge to perform the essential functions of the job.
- Must possess Qualified Mental Health Associate certification, provided through the Mental Health and Addiction Certification Board of Oregon at the time of appointment.
- Must have and maintain a valid driver's license and automobile insurance as a condition of employment.
