About the Role We are seeking a permanent, dedicated Hospice RN Case Manager to join our compassionate care team in Brookings, Oregon . In this role, you will provide end-of-life care, symptom management, and emotional support to patients and their families. Because our service area spans counties on both sides of the state line, this position uniquely services communities in both Southern Oregon and Northern California . Key Responsibilities Case Management: Coordinate and oversee the complete hospice care plan for a dedicated caseload of patients. Clinical Assessment: Manage complex physical symptoms, administer medications, and provide skilled nursing interventions. Interdisciplinary Collaboration: Work alongside physicians, social workers, spiritual counselors, and home health aides to deliver holistic care. Documentation: Maintain accurate, timely electronic charting regarding patient status and care plans. Family Support: Provide vital education, emotional reassurance, and guidance to families and caregivers. Qualifications & Requirements Experience: Verifiable, professional hospice nursing experience is strictly required . Licensure: Must hold active, unrestricted Registered Nurse (RN) licenses in both Oregon and California (or have immediate eligibility to practice in both states).
