Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
\n
• Active State License is Required to Start the Assignment
\n
• BLS Certification May Be Required from AHA or ARC
\n
• Home health experience
\n
• Experience with HCHB
\n
• Associates Degree
\n

Assignment Details

\n
• Shift schedule: Mon-Fri am to 4:30 pm
\n
• No on-call or weekend requirement
\n
• Coverage areas include Logan and Todd
\n
• Mileage reimbursed between visits, from first patient to last patient
\n
• Responsibilities include performing Oasis assessments using HCHB, supervising PTA, and completing admissions, recerts, 30 day assessments, and discharges
\n

Location Highlights

Russellville offers access to western Kentucky landmarks and a quieter small-city setting for a home health assignment. Downtown Russellville features the Logan County Courthouse and is about 30 miles from Bowling Green, which provides additional dining, shopping, and entertainment options. Lake Malone State Park is roughly 45 miles away and offers hiking, fishing, and boating, while Mammoth Cave National Park is about 70 miles northeast for day-trip outdoor recreation. The area also supports regional travel with access to U.S. Route 68 and proximity to Nashville, which is about 50 miles south and adds airport, music, and cultural attractions.
