In this role, you will support the Kidney/Pancreas Transplant Program by conducting comprehensive psychosocial assessments to determine candidacy for kidney and/or pancreas transplantation and presenting your recommendations to the multidisciplinary Selection Committee. You will provide ongoing support and care coordination to patients on the transplant wait list as they prepare themselves and their support systems for transplantation.The ideal candidate will have experience in conducting psychosocial assessments, group facilitation, crisis intervention, and interprofessional collaboration. They will also have experience working with patients and families presenting with a wide range of mental health and psychosocial concerns including anxiety and depression, adjustment and coping, loss and bereavement, complex family dynamics, substance use disorder and trauma. Excellent clinical, communication, and documentation skills are a necessity.SummaryThis position will collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.Does this position require Patient Care? YesEssential FunctionsProvides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues.
