Kaiser Permanente is seeking an LMSW Case Manager, Social Work to support inpatient services in Lanham, MD. In this role, you will provide comprehensive psychosocial assessments, develop care plans, and coordinate discharge planning for diverse patient populations. You will collaborate closely with physicians, nurses, and interdisciplinary teams to ensure safe transitions of care and access to community resources. Responsibilities include crisis intervention, patient and family education, advocacy, and meticulous clinical documentation.
