We are currently seeking a Licensed Social Worker (LSW) to join our dynamic team and contribute to our mission of delivering exceptional care and support to our clients and their families. Job Duties and Responsibilities Conduct comprehensive assessments of clients' social, emotional, and psychological needs. Develop, implement, and evaluate care plans in collaboration with a multidisciplinary team. Provide counseling and support to clients and their families, addressing social and emotional concerns. Liaise with community resources and external agencies to coordinate additional support services. Advocate for clients' rights and needs within the facility and in the broader community. Participate in team meetings and contribute to the development of policies and procedures related to social work and client care. Required Qualifications and Skills Bachelor's or Master's degree in Social Work from an accredited institution. Valid Licensed Social Worker (LSW) certification. Demonstrated experience in a healthcare setting, preferably in a care facility or similar environment. Strong interpersonal and communication skills, with the ability to work effectively with diverse clients and teams.
