Key Responsibilities of the RN Case Manager

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers for comprehensive care plans
• Identify and reassess appropriate levels of care
• Coordinate non-medical support for treatment compliance
• Facilitate care transitions with multidisciplinary teams
• Engage specialty resources to achieve optimal outcomes
• Document clinical, functional, and fiscal outcomes accurately
• Promote health and illness prevention strategies
• Act as a patient advocate, ensuring privacy and confidentiality

Requirements of the RN Case Manager

• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted RN license in Alabama
• Case Management certification
• Ability to analyze complex medical information
• Experience with home healthcare, OASIS, and HCHB
