Complex Care Coordinator RN (Weekdays M-F)

Job Summary The RN Complex Care Coordinator is responsible for assessing complex patient transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure care needs are met. The role serves as an expert resource and consultant in clinical, psychosocial, and resource matters, developing safe and timely discharge plans and collaborating with the interdisciplinary team. Responsibilities Conduct comprehensive clinical and psychosocial assessments for high‑risk patients, including those at high risk of readmission, high cost, long stay, or difficult to place. Develop and implement timely and appropriate discharge plans, ensuring safe transition to the next level of care. Coordinate and facilitate post‑acute referrals and community resource connections, including indigent services, housing, social referrals, and elder assistance. Advocate for patient and family autonomy in health‑care decision making and address cultural or religious beliefs. Participate in interdisciplinary rounds to confirm estimated discharge dates and recommend appropriate care transitions. Identify and document barriers to timely disposition and work with private, public, state, and federal resources to address them. Assist in negotiating agreements with community agencies and facilities. Precept and mentor new and less senior care coordinators, providing case reviews and guidance on complex scenarios. Support professional development initiatives and contribute to protocol and performance‑improvement activities. Ensure accurate and timely documentation in the electronic health record, including psychosocial assessments and service arrangements. Qualifications Minimum of 3 years of experience in acute‑care healthcare settings. Minimum of 2 years of experience in care coordination within an acute‑care environment. Strong written and verbal communication skills, with maturity, confidence, and a positive attitude. Self‑directed, able to function under stress, handle change, and thrive in a fast‑paced environment. Excellent assessment, interview, organizational, and problem‑solving skills. Knowledge of local, state, and federal regulations, and community and statewide resources and programs. Ability to collaborate effectively with physicians, care team members, and patients/families. Education & Licensure Associate's degree in Nursing or a nursing diploma, or a Bachelor's degree in Nursing (preferred). Required certifications upon hire: BLS (Basic Life Support) or equivalent; RN–Regular (Single State) or RN‑COMPACT (Multi‑state Compact). Other certifications may be required depending on practice setting. Skills and Competencies Expertise in complex patient care coordination and discharge planning. Proficiency in using electronic health record systems. Strong interpersonal skills to manage sensitive situations such as end‑of‑life planning, financial and legal concerns, and social determinants of health.
