Medical Social Worker

Oak Street Health seeks a Medical Social Worker LMSW to support Medicare patients in underserved communities. In this role, you’ll complete psychosocial assessments, coordinate care plans, and connect patients and families with community resources. You will collaborate closely with physicians, nurses, and care teams to address social determinants of health, provide counseling and crisis intervention, and support safe care transitions.
