Join our dedicated team as a Registered Nurse specializing in diabetes care at Tsehootsooi Medical Center in Fort Defiance, Arizona. This vital role involves providing comprehensive diabetes-related services within both the Diabetes Clinic and Primary Care Clinic settings.

The successful candidate will be responsible for expert consultation, thorough assessment, and ongoing evaluation of diabetes services for individuals, families, and the broader community. This position requires a commitment to patient education and the application of strong clinical judgment to support optimal patient outcomes in diabetes management.

Key Responsibilities

• Provide specialized consultation and education on diabetes management to patients, families, and community members.
• Conduct comprehensive assessments and evaluations of diabetes-related services and patient needs.
• Develop and implement individualized care plans to support patients in managing their diabetes effectively.
• Utilize strong critical thinking and triage skills, including knowledge of the Emergency Severity Index (ESI), to prioritize patient care.
• Collaborate effectively with interdisciplinary healthcare teams to ensure coordinated and holistic patient care.

Required Qualifications

• Minimum of three (3) years of recent, direct experience in a clinic setting or diabetes specialty.
• Mandatory clinic experience.
