Licensed Vocational Nurse

Join a patient-centered healthcare team as a Licensed Practical / Vocational Nurse (LPN/LVN). Key Responsibilities Include Providing skilled nursing care under supervision, following care plans and physician orders. Assessing patient conditions, documenting activities, and updating care plans. Monitoring for changes in patient status and implementing appropriate interventions. Educating patients and families on disease management, self-care, and prevention. Ensuring infection control and compliance with regulations and standards. Qualifications Active, unrestricted LPN/LVN license in the practicing state. Current CPR certification and TB test within the past 12 months. Preferred: six months of clinical nursing experience, home health experience, and proficiency in assessments and documentation. Must have reliable transportation and physical ability to lift and reposition patients. Skills & Abilities: attention to detail, time management, problem-solving, effective communication, and organization.
