\nJob Summary:\n
\n
\nProvides social work support to patients in their place of residence. Responsibilities include providing community resources, placement options and counseling to patients and families as needed. Acts as a competent clinician who provides clinical care in a skilled and effective manner, utilizing appropriate technical and decision-making skills.\n
\n
\nJob Requirements:\n
\n
\nEducation and Work Experience:\n
\n

\n
• Bachelor's Degree or equivalent (some certifications will require Bachelor's): Required
\n
• Master's Degree in social work or equivalent combination of education/experience: Required
\n
• Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare: Preferred
\n

\n
\nLicenses/Certifications:\n
\n

\n
• Licensed Clinical Social Worker (LCSW) in state of practice: Preferred
\n
• Valid Driver's License (DL) and must be at least 21 years of age or older: Required
\n

\n
\nEssential Functions:\n
\n

\n
• Makes the initial patient assessment as ordered.
\n
• Documents assessments, visits, and contacts on patient's chart.
\n
• Places phone calls and makes visits as needed, serving as a resource liaison when appropriate.
\n
• Provides patients/families with community resources.
\n
• Attends weekly team conferences.
\n
• Performs other job-related duties as assigned.
\n
