Greek American Rehab & Care Center is seeking a dedicated and compassionate Full-Time Occupational Therapist (OT) to join our rehabilitation team. The Occupational Therapist will provide high-quality evaluation, treatment, and rehabilitation services to residents with the goal of improving independence, functional abilities, safety, and quality of life.
Essential ResponsibilitiesEvaluate residents' functional abilities and develop individualized occupational therapy treatment plans.
Provide skilled occupational therapy interventions based on each resident's needs and established plan of care.
Address activities of daily living, mobility, coordination, strength, balance, cognition, and other functional skills.
Monitor residents' progress and modify treatment plans as appropriate.
Document evaluations, treatments, progress, and discharge information accurately and timely.
Collaborate with physicians, nurses, physical therapists, speech therapists, nursing assistants, social services, and other members of the interdisciplinary team.
Educate residents, families, and caregivers regarding treatment programs, adaptive techniques, and safety strategies.
Maintain compliance with applicable federal, state, and facility regulations and professional standards.
Participate in care planning and interdisciplinary meetings as needed.
Promote a safe, respectful, and supportive environment for all residents.
Current Occupational Therapist license in the State of Illinois.
Graduate of an accredited Occupational Therapy program.
Current CPR certification, if required by facility policy.
Experience in skilled nursing, rehabilitation, long-term care, or a related healthcare setting preferred.
Strong communication, organization, documentation, and interpersonal skills.
Ability to work effectively as part of an interdisciplinary healthcare team.
Compassionate, dependable, and committed to providing resident-centered care.
