Quick Facts

• Covers 3 locations per day approx. 6 locations per week
• Total patient panel of 340 patients across 8 locations
• Mix of short-term and long-term residents ages 55 and older
• New patient admissions with 30, 60, and 90-day follow-ups
• Part of a Patient Aligned Care Team (PACT) model
• Chronic disease management required HTN, DM, CHF, COPD, CKD
• GI management required GERD, constipation, malnutrition, incontinence
• Geriatric post-acute care experience required
