Empower patients and families to reach healthcare goals by educating them on disease management. Case manage and provide clinical oversight to interdisciplinary team. Requirements RN license in the state of work. Two years of experience as a registered nurse; home health experience preferred. Current driver’s license. Current CPR and BCLS certification.
