Hours: 40 hours per week
Estimated Weekly Pay: $1,810 - $1,940

Job Requirements

Active Physical Therapist (PT) license or eligibility to practice in Wisconsin
At least 12 months of home health experience within the past 3 years
Minimum 1 year of relevant clinical experience required
Experience working with home health documentation and patient care plans
Familiarity with HCHB preferred
Strong communication, organization, and time-management skills

Responsibilities

Evaluate patients in the home setting and develop individualized treatment plans
Provide skilled physical therapy interventions to support mobility, safety, and functional progress
Track patient outcomes and adjust care plans as needed
Educate patients and caregivers on therapy goals, exercises, and home safety strategies
Document visits accurately and timely in the EMR system
Collaborate with the care team to help ensure coordinated, high-quality patient care
