This opportunity is for a Diabetes Pharmacist role based in Lawton, OK, with some required travel to nearby Carnegie and Anadarko. The position is a 1-year contract with the possibility of four 1-year extensions, starting as soon as possible. The schedule includes 8-hour day shifts, offering a steady work routine.

Key Responsibilities:

• Process daily prescription orders, fill and dispense new prescriptions and refills, and provide extensive patient counseling.
• Counsel patients specifically on medication management.
• Oversee pharmacy support personnel to ensure efficient pharmacy operations.
• Practice intensive diabetes management under a collaborative practice agreement.
• Utilize evidence-based guidelines, including American Diabetes Association Standards of Care, to make informed decisions in diabetes treatment.
• Demonstrate proficiency in placing continuous glucose monitors (CGMs), extracting data, and interpreting it to adjust medications properly.

Qualifications and Desired Experience:

• Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD).
• Valid state pharmacy license.
• Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) from the Board of Pharmacy Specialties, or the completion of a Post-Graduate Year One residency.

Location:

• Lawton, OK, with some required float to Carnegie and Anadarko.
