Speech Pathologist I, Hoag at Home provides speech therapy services to patients in the comfort of their own homes. This is a great opportunity for a Speech-Language Pathologist who enjoys working independently, building strong relationships with patients and families, and helping individuals improve communication, cognitive, and swallowing function in their everyday environment. As part of the Hoag at Home team, the Speech Pathologist partners closely with patients, caregivers, physicians, nurses, therapists, and other members of the care team to develop individualized treatment plans that support safety, independence, and quality of life. Responsibilities Complete comprehensive in-home evaluations for patients with speech, language, cognitive, communication, and swallowing needs. Develop individualized plans of care based on the patient’s functional needs, goals, and home environment. Provide skilled speech therapy interventions and monitor patient progress throughout the episode of care. Educate patients, families, and caregivers on therapy goals, home exercises, equipment, safety, and strategies to support continued progress. Perform regular reassessments and adjust treatment plans as needed. Collaborate with physicians, nurses, therapists, and other members of the multidisciplinary care team. Make recommendations regarding ongoing therapy needs and discharge planning. Manage a caseload of home health patients while maintaining timely and accurate documentation. Complete all required documentation in accordance with professional, regulatory, and organizational standards. Participate in continuing education and incorporate current best practices into patient care. Perform other duties as assigned. Qualifications Education and Experience Master’s degree or equivalent in Speech-Language Pathology from an accredited educational institution required. Experience working with patients with communication, cognitive, and/or swallowing disorders preferred. Home health, acute care, rehabilitation, or other clinical speech therapy experience preferred. Experience with Microsoft Office and healthcare database systems preferred. Bilingual or multilingual abilities preferred. Ability to work independently while collaborating effectively with an interdisciplinary care team. Strong communication, organization, time management, and patient education skills. Licensure Current valid Speech-Language Pathologist license in the State of California required. Certification Current American Heart Association Basic Life Support (BLS) Certification required. #J-18808-Ljbffr Hoag Health System
