Hospice Registered Nurse

Hospice RN Join our team at Buckeye Hospice and Palliative Care Services in Zanesville, Ohio as a compassionate and dedicated Registered Nurse (RN) Case Manager. As a Case Manager (RN), you will play a crucial role in providing high-quality care to our patients while promoting their independence, dignity, and quality of life. Job Summary: We are seeking a Registered Nurse (RN) to join our team as a Case Manager at Buckeye Hospice and Palliative Care Services in Zanesville, Ohio. The ideal candidate will have a passion for delivering patient-centered care and be committed to making a difference in the lives of those we serve. Responsibilities: Provide initial and comprehensive assessments of patient needs, developing personalized care plans to address their health and social needs. Implement and revise care plans as needed, ensuring that patients receive the best possible care and services. Communicate effectively with patients, families, and healthcare teams to coordinate care delivery and ensure seamless transitions. Supervise and instruct direct care staff members and non-professional hospice aide staff to deliver quality care. Participate in on-call rotation, providing 24/7 coverage for patients and families in need. Requirements: RN License is REQUIRED. Valid Driver's License and reliable transportation that is insured in accordance with Ohio state requirements. Minimum of 1-4 years of experience as a Registered Nurse working with hospice services or home health agency. On-Call Rotation. Work Environment: Multiple locations in Zanesville and surrounding areas. Long-term care settings, including nursing facilities and home settings. Monday to Friday, with occasional on-call and weekend availability.
