Client Care Coordinator
BrightQuest Treatment Centers in La Mesa, CA is hiring a Client Care Coordinator to join their team! The Client Care Coordinator will work collaboratively with the Treatment Teams, the Clinical Teams, and Behavioral Health Advisors to assure care planning and coordination begins at the start of treatment and continues through to the completion of the aftercare plan. The Client Care Coordinator will also work with providers and referents to ensure good quality customer service, communication, and collaboration in the development of aftercare plans. This position will require individual meetings with clients to determine their needs for ancillary services while in treatment, as well as aftercare planning, presence in team meetings, presence in clinical treatment team meetings, and presence in discharge planning meetings.
Qualifications
- Bachelor's degree in Social Services or related field required OR two years or more experience working in the Behavioral Health field and/or with dually diagnosed clients in a residential setting.
- Strong documentation skills with competency in the use of the electronic health record; assessment and evaluation skills; internal communications; and reporting skills.
- Ability to work with diverse team membership (including nursing, psychiatry and therapists, referents, and outside providers).
- Exceptional organizational and time management skills.
- Specific, required, current, and active licensure, registration, and/or certification must be maintained in good standing.
- Sound understanding of mental illness, co-occurring disorders, and milieu therapy.
- Proficient computer skills and ability to use various software programs.
- First Aid/CPR certified.
- Have a valid driver's license in good standing and auto insurance.
- Successfully complete comprehensive background check (including criminal record clearance), MVR screen, and drug screen.
- Successfully complete required health screening, including a current and valid TB clearance.
- Ability to multi-task in a fast-paced work environment.
- Flexibility to work evenings, nights, and weekends, and adapt schedule as needed.

- Works with the Admissions team in obtaining requested treatment records for incoming clients as necessary. Provides treatment records in response to incoming requests with appropriate authorization. Transmits treatment records to continuing care providers, referents, feedback professionals, etc. at the conclusion of each client's treatment. Prepares and distributes copies of treatment records in compliance with HIPAA Privacy Laws.
- Along with Therapist, communicates with members of the Behavioral Health Advisor team upon each new client's admission and through treatment to identify appropriate aftercare resources, ensure that referent preferences are incorporated, and resolve needed elements prior to discharge.
- Along with Therapist, communicates with each client's identified referral source within the first week following admission to identify the referent's preferences/recommendations for the continuing care plan, or any resources that we will need to identify to supplement. Updates referent as continuing care planning progresses. Communication with the referent comes after contact with the BHA to ensure unity in teamwork and coordination.
- Attends assigned treatment team meetings to track specific continuing care needs, which are then addressed through internal coordination and contact with the BHA and/or referent as appropriate.
- Facilitates client survey collection and other case management duties as assigned by the Treatment Team during the treatment episode, if applicable.
- Meets with clients and/or assigned therapists at a determined frequency from admission to discharge to facilitate internal care coordination needs as well as continuing care plan development.
- Supports client, staff, and medical providers in coordinating any necessary off-site ancillary appointments while client is residing in treatment, if necessary.
- Supports client with researching and considering continuing care recommendations as presented by the treatment team.
- Works with therapists to obtain client commitment to recommended plan elements, coordinates all communications needed between client and future providers to support smooth transitions in care, books appointment times prior to discharge, and documents all referrals and agreed-upon plan elements in the Aftercare Plan document and Clinical Discharge Summary.
- Facilitates group sessions for clients on a regularly-scheduled and/or as-needed basis.
- Responsible for regularly scheduled facility reporting of referrals provided to each discharged client.
- Coordinates with BHA team to maintain databases that track referrals, feedback from referring facilities, and resource lists and libraries.
- Partners with other staff as needed with regard to discharge planning and other projects.
- Participates as a team player in the milieu, working harmoniously with other staff and contributing to strong clinical care and client support.
- Communicates with other staff in relaying concerns and observations involving client safety issues.
- Utilizes supervisory feedback to improve performance.
- Knowledge of the philosophy and approach of the 12-step model of recovery (AA, CA, NA, Al-Anon, etc.)
- Fully understand and maintain policies regarding professional ethics, including appropriate boundaries and client confidentiality; monitor documents according to company and HIPAA policies.
- Professionally, ethically, and compassionately relates to clients, families, referral sources and staff at all times.
- Meet timeliness expectations of completing work as scheduled, demonstrating self-paced performance and adjusting priorities as needed.
- Meet quality expectations of accuracy, completeness, and responsiveness to feedback.
- Write clear and concise statements summarizing client's feelings, presentation and response in clinically significant encounters.
- Model appropriate interpersonal relationships, emotional regulation, and boundaries.
- Model and reinforce positive behaviors of clients, and promotes social interaction by conversing with them, participating in activities with them, etc.
- Knowledge of current legal standards pertaining to professional practice, reporting requirements, professional boundaries, and relate issues.
- Reports any immediate safety hazards to appropriate supervisory personnel or maintenance personnel.
- Report resident abuse and neglect.
- Attend and participate in assigned and required trainings, supervisions, and weekly meetings.
- Ability to demonstrate awareness and respect for cultural, lifestyle, age, gender, and sexual orientation diversities of clients and staff; apply knowledge of age, gender, and multi-cultural counseling skills.
- Understands and adheres to CBH's compliance standards as they appear in CBH's Ethics and Code of Conduct Policy and Conflict of Interest Policy.
- Keeps abreast of all pertinent federal, state and CBH regulations, laws, and policies as they presently exist and as they change or are modified.
- Performs other related duties as assigned.
