Key Responsibilities:

• Evaluate patient needs and create individualized treatment plans
• Deliver therapy interventions and monitor outcomes
• Educate patients and caregivers on recovery goals
• Maintain accurate clinical documentation
• Work collaboratively across interdisciplinary teams

Requirements

Requirements

• Active Massachusetts PT license or ability to obtain prior to start
• Graduate of an accredited Physical Therapy program
• Current BLS certification preferred
• SNF experience preferred
• Strong communication and clinical skills
