If you’re passionate about making a difference during life’s most tender moments, we invite you to apply and become part of a mission-driven team rooted in purpose and heart.Position SummaryThe Nurse Practitioner's primary functions are to assist in overseeing the clinical care and treatment of patients and families; to consult with the patients' attending physician, in consultation with the hospice physician; provide clinical direction to the Interdisciplinary Group/Team to ensure quality care.Key ResponsibilitiesCollaborate with the IDG/T in assuring appropriate evaluation and palliative treatment of terminal disease.Consult with and assist in the IDG/T's development of patient/family plan of care to assure quality care is provided.Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.Perform and document visits to patient/family based on plan of care and patient/family need.Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.Consult with and provide education to other IDG/T members on a prn basis for clinical issues relating to palliation.Actively participate in the IDG/T. Communicate effectively, timely, and appropriately at all times.Inform physicians and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care.Complete face-to-face encounters with patients, as assigned, prior to 3rd benefit period and each period afterwards.Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.Always uses standard precautions, and per hospice policy.Adheres to hospice policies regarding protected health information.Keep up with and maintains education/knowledge in pharmacology and palliative medicine.Provide on-call services to patients/families as assigned.QualificationsGraduate from a Baccalaureate School of Nursing, a Nurse Practitioner (NP) focused Master's or Doctoral Nursing ProgramCurrent state license as a Registered Nurse, and as an Advanced Practice Nurse. Current driver's license.Two years' experience as an Advance Practice Nurse, clinical care setting preferred; hospice or home health preferred.Nursing skills as defined as generally accepted standards of practice. Comprehensive knowledge of pharmacology, palliative care, and end-of-life. Good interpersonal skills.Transportation: Reliable transportation and valid auto liability insurance.Ability to work a flexible scheduleAbility to travel locallyAbility to remain calm in stressful conditions and make quick decisions to meet patient and family individualized psychosocial needs.
