RN Care Manager

Mercy is seeking a part-time Weekend RN Care Manager to coordinate high-quality, patient-centered care across settings. In this role, you will perform comprehensive assessments, develop and implement individualized care plans, and collaborate with physicians, therapists, social workers, and families to ensure safe, effective transitions of care. You will monitor patient progress, support discharge planning, and advocate for resources and services.
