A-Line Staffing is now hiring a Full-Time, Field Based / Remote, Pediatric Home Health Case Manager in Bexar and surrounding counties! (MUST USE YOUR OWN TRANSPORTATION, MILEAGE IS REIMBURSED).

Position Summary

We are seeking an experienced Registered Nurse (RN) to join the team as a Senior LTSS Service Care Manager. This field-based role supports children enrolled in the a program by conducting comprehensive assessments to identify unmet healthcare needs and evaluate eligibility for Medicaid services and supports.

The ideal candidate is highly organized, self-motivated, and comfortable working independently while collaborating within a remote team environment. This role combines member-facing field work with extensive documentation responsibilities and requires strong clinical assessment and case management skills.

Pediatric Home Health Case Manager Compensation

• The pay for this position is $42.00/hr + mileage
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Pediatric Home Health Case Manager Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position M-F 8am-5pm with occasional overtime

Typical Work Schedule

• Approximately 6 member assessments per week.
• Typically, 3 days conducting two member visits per day.
• Approximately 2 dedicated documentation days each week.
• Travel is required from the employee's home office to member homes and back.
• Pediatric appointments are often scheduled after school hours (e.g., 3:00 PM), which may occasionally extend beyond normal business hours.

Pediatric Home Health Case Manager Responsibilities

·         Conduct initial assessments for pediatric Medicaid members.

·         Complete in-home member visits throughout assigned territory.

·         Assess medical, behavioral, social, and support service needs.

·         Identify gaps in care and recommend appropriate Medicaid services and community resources.

·         Develop and maintain comprehensive care management documentation.

·         Ensure all assessments and documentation are completed within established turnaround times.

·         Maintain compliance with organizational, regulatory, and training requirements.

·         Collaborate with interdisciplinary teams and internal stakeholders.

·         Manage scheduling, travel, documentation, and member interactions effectively.

Pediatric Home Health Case Manager Requirements

• Associate Degree in Nursing (ADN), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN)
• Active Registered Nurse (RN) license
• Minimum 3+ years of nursing experience
• Minimum 2+ years of case management experience

Preferred Qualifications

• Home Health experience
• Pediatric nursing experience
• Medicaid or managed care experience
• Experience conducting in-home assessments

Additional Requirements

• Ability to travel throughout San Antonio, Bexar County, and neighboring counties.
• Reliable transportation for in-home member visits.
