Clinical Social Worker

Silver Rising Therapy is seeking a compassionate, patient, and creative LCSW, LCPC, or LMHC to provide on-site emotional support and cognitive stimulation to older adults living in a dedicated memory care community. About the Role This is a unique clinical role designed specifically for individuals living with dementia, advanced cognitive decline, and age- or end-of-life-related needs . While traditional therapy may be appropriate for some residents, much of this work is focused on meeting residents where they are cognitively and emotionally . The goal is not always traditional therapeutic progress. Often, success means creating a moment of connection, reducing anxiety or distress, providing comfort, encouraging engagement, or helping a resident have a better day. Depending on the individual, sessions may incorporate: Music, familiar songs, photographs, or reminiscence Cognitive stimulation and simple games or activities Validation, redirection, and supportive conversation Emotional support surrounding grief, loss, aging, and life transitions Comfort and companionship for residents with advanced dementia or limited verbal communication Support for residents and families navigating cognitive decline and end-of-life transitions Some residents may be highly conversational, while others may have significant cognitive impairment or be minimally verbal or nonverbal. Sessions are adapted to each resident's abilities and may range from shorter 15-30 minute visits to more traditional therapy sessions. Patience, warmth, flexibility, and the ability to connect with residents in nontraditional ways are essential to success in this role. Silver Rising Therapy provides: An established on-site caseload within one senior living community The opportunity to build a full-time caseload , if desired Flexible self-scheduling during typical weekday hours Client referrals and insurance coordination Bi-weekly compensation EHR and documentation training Ongoing clinical resources and support A collaborative network of clinicians working with older adults Because your caseload is concentrated within one community, you can move from resident to resident throughout the day without driving between individual appointments. Role Responsibilities Provide individualized emotional support, cognitive stimulation, and therapeutic services to memory care residents Adapt sessions to each resident's cognitive abilities, communication style, mood, and needs Find creative ways to engage residents who may have difficulty participating in traditional talk therapy Conduct initial and ongoing evaluations and develop individualized treatment plans Provide individual and group services when appropriate Maintain accurate and timely clinical documentation Collaborate with community staff and interdisciplinary care teams Communicate with families and POAs when appropriate Support residents navigating dementia, grief, loss, anxiety, depression, aging, and end-of-life transitions Maintain access to our EHR system via laptop Who Thrives in This Role The right clinician does not necessarily need extensive memory care experience. More importantly, we're looking for someone who is: Exceptionally patient and compassionate Comfortable working with individuals with significant cognitive impairment Comfortable engaging residents who may be minimally verbal or nonverbal Creative and willing to think beyond traditional talk therapy Able to find meaning in small moments of connection and engagement Flexible when a resident's abilities or needs change from day to day Comfortable working independently while collaborating with community staff Passionate about improving quality of life for older adults Previous experience in memory care, dementia care, hospice, palliative care, skilled nursing, assisted living, geriatrics, or end-of-life care is highly valued but not required. Qualifications Master's degree in Social Work, Counseling, Psychology, or related field Active independent state licensure as an LCSW, LCPC, LMHC, or equivalent Clinical experience preferred Senior care, memory care, hospice, or geriatric experience preferred Strong interpersonal and communication skills Ability to work independently and manage your own schedule Commitment to ethical practice and high-quality resident care Proof of independent licensure and malpractice insurance Ability to pass a background check Why Silver Rising Therapy?
