Job Specification:

• Deliver skilled nursing care and education to patients under the direction of a registered nurse and physician.
• Monitor patient progress, document care using EMR systems, and report changes in condition.
• Collaborate with multidisciplinary team members to ensure coordinated and effective patient care.
• Provide compassionate, patient-centered support in a home setting, adhering to safety protocols and care standards.

Candidate Requirements:

• Valid LPN license in New York
• Current BLS Certification
• New graduates are welcome to apply
• Familiarity with EMR systems and basic computer proficiency
