Requirements:

• Active and unencumbered Registered Nurse (RN) license in Texas or compact state license
• Minimum of 1 year acute care case management experience required
• Strong knowledge of discharge planning, utilization review, and care coordination
• Experience with Meditech and Careport EMR systems preferred
• Certified Case Manager (CCM) credential is a plus
• Self-starter with solution-focused approach and collaborative mindset
