Registered Nurse Home Health

Shift: 8 hours

Contract Length: 12 weeks

Guaranteed Hours: 40

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience
• Current CPR certification
• Current driver's license
• Reliable transportation and insurance

Roles & Responsibilities:

• Administer skilled nursing care for clients of all ages in their place of residence.
• Assess home health and/or hospice patients to identify physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and/or their families regarding care, medications, and home safety.
• Supervise and provide clinical direction to nursing assistants to ensure quality service.
• Perform on-call responsibilities as assigned and provide effective communication among team members and healthcare professionals.
