As our Home Care Client Care Manager , you will be the heart of our client experience. Your mission is to ensure every client receives compassionate, high-quality support and every family feels confident, informed, and cared for.
Your responsibilities will include:
• Developing, maintaining, and overseeing personalized client care plans
• Building trust and relationships with clients, families, caregivers, and referral sources
• Mentoring caregivers and supporting meaningful, safe, exceptional care delivery
• Acting as a calm, positive, solutions-focused leader—no matter how busy the day gets
• Playing an essential role in helping prospective clients become lifelong Right at Home clients
• Supervising, on a rotation, our on call care coordinators after hours
• Supporting community-facing efforts as needed to strengthen our referral partnerships
