Remote RN Care Coordinator: Chronic Care Navigator
Stryker Orthopaedics
UnitedHealth Group is seeking a Care Coordinator to manage a panel of members with chronic needs, focusing on medical, behavioral, and social drivers of health. Remote-friendly within Idaho, you will support care plans, coordinate with the care team, and help members access services and resources.
The role requires Idaho licensure (RN or LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT), at least 1 year of Medicaid/Medicare or SDoH experience, and proficiency with MS Office; travel up to 50% may be needed.
