Overview

Support high-quality patient care as a Licensed Practical Nurse working under the direction of a Registered Nurse. In this role, you will assist with implementing the nursing plan of care by monitoring patients, observing conditions, and reporting changes or responses to treatment. You will also perform nursing tasks within your scope of practice, helping ensure safe, reliable , and compassionate care. Responsibilities

• Deliver hands-on patient care within scope of practice, including administering medications, supporting comfort needs, and assisting with treatments and interventions.

• Contribute to admission assessments by collecting and documenting patient data, and collaborate with RNs and the care team to implement care plans, monitor progress, and support safe, high-quality outcomes.

• Provide input during rounding and communicate changes in patient condition promptly to ensure timely clinical response.

• Support safe care transitions by maintaining accurate , real-time documentation, participating in hand-offs, and communicating patient progress with the healthcare team.

• Collaborate with RNs and providers to update care plans and ensure continuity of care across the hospitalization .

Qualifications

Education:

• Graduate of accredited LPN program required .

Experience:

• One (1) year of previous CNA/PCT experience preferred.

License(s)/Certification(s):

• Valid LPN license in the state(s) of practive including eligibility under an NLC multistate license required .

• Iowa Board of Nursing (IABON)

• Illinois Department of Financial & Professional Regulation (IDFPR)

• Wisconsin Department of Safety and Professional Services (WIDSPS)

• Dual licensed positions: Team members must obtain and submit proof of dual licensure within 90 days of hire/transfer. Proof of application is acceptable during this period – required within 90 days of hire.

• Must possess and maintain current Basic Life Support (BLS) certification through the American Heart Association required .

• National Institute of Health Stroke Scale-English preferred:

• National Institute of Health (NIH) Stroke International

• Advanced De-escalation Training preferred:

• Management of Aggressive Behavior Training International Inc (MOAB)

• Crisis Prevention Institute (CPI)

• Uniform Data Systems Med-Rehab for Rehab units (UDSMR)

• IA Child Abuse Mandatory Reporter Training in state(s) where providing care  required .

• I L Child Abuse Mandatory Reporter Training in state(s) where providing care  required .

• IA Adult Dependent Abuse Mandatory Reporter Training in state (s) where providing care   required .

• I L Adult Dependent Abuse Mandatory Reporter Training in state (s) where providing care   required .
