VitalCore Health Strategies – Telehealth Coordinator / Chronic Care Registered Nurse Full‑time telehealth coordinator at Howard R. Young Correctional Institution, Wilmington, DE. VitalCore Health Strategies (VCHS) is an industry leader in correctional health care and is seeking a registered nurse to coordinate telehealth services and manage chronic care for patients within the facility. You will monitor patients from intake through release to the community, ensuring continuity of care and acting as a liaison among the multiple systems impacting the lives of offenders. Qualifications Graduate of an accredited School of Nursing Active licensure as a Registered Nurse in Delaware or a multistate license Active CPR certification Knowledge of specific state laws and regulations governing practice Completion of initial and annual clinical competencies as assigned Essential Functions Maintain accurate, detailed reports and records Document medical histories, test results, treatment plans, and communication with external providers Administer medications and monitor for reactions and side effects Monitor, record, and report symptoms or changes in patients’ condition Coordinate with outside healthcare providers, schedule appointments, and ensure proper medical records are prepared Perform administrative tasks such as scheduling, record‑keeping, and coordination with external providers Additional duties as assigned by the facility Schedule Day Shift – 8‑hour #J-18808-Ljbffr VitalCore Health Strategies
