Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that meet the unique needs of each resident
Monitor and report changes in residents' conditions, communicating effectively with families and healthcare professionals
Participate in quality improvement initiatives to ensure optimal outcomes and resident satisfaction
