LPN PositionHigh volume medical group seeking FULL-TIME LPN to provide direct and indirect care for patients. Must have the ability to handle administrative and clinical duties including patient screening and brief assessment of problems, medication review, full vital signs, injections, screening services, and other provider supervised treatments.Essential functions and basic duties:Performing Annual Wellness Visits (AWVs)Assist in quality and gap improvements programWorking patient gap outreach reportsCollaborate with providers and educate patientsCoordinate of care for patientsAdminister adult and child vaccinesRefill prescriptionsRunning flu clinicsTriage phone calls for physicianSchedule appointments and respond to patient messages and follow through to resolutionResult call backsVerify patient information, records medical history, confirming purpose of visitPrepares patients for examination by performing preliminary tests, taking height, weight, blood pressure, respirations, pulse, O2 and, when necessary, HgbA1c, glucose, and urinalysis for acute diagnostic testing or drug screeningMaintains cleanliness of exam rooms any other workspace providedDevelop relationships with staff, providers and patients in ways that demonstrate caring and reflects professional behaviorPerforms related work assignments and special projects as assigned in the allotted time frame and reports any delaysAttends and participates in office meetings as scheduledCompletes assignments in a timely manner stays informed of all changes in healthcare and shares these changes with practice managerLeads and complies with PCMH implementation and standardsComplies with all HIPAA policies and procedures ensuring confidentiality and patient privacyComplies with OSHA/CLIA rules and regulationsComplies to all company policies and office proceduresWe offer a full package of benefits including medical, voluntary dental, vision, life insurance, short and long-term disability. Working knowledge of Microsoft products; Excel, Word.At least 2 years or more experience
