Responsibilities

• Provide comprehensive physical therapy assessments and develop individualized treatment plans.

• Deliver evidence-based PT treatments to improve mobility and function.

• Meet or exceed defined productivity targets and contribute to pipeline visibility.

• Maintain compliance with licensure, safety, and organizational policies.

• Monitor patient progress and adjust plans to maximize outcomes.

• Document care plans, progress notes, and outcomes in electronic health records.

• Collaborate with a multidisciplinary team to coordinate patient care.

• Participate in ongoing professional development and apply new skills.

• Engage in on-site patient care with a results-oriented mindset and measurable outcomes.

• Educate patients and caregivers on exercises, safety, and home programs.

• Identify opportunities for process improvements to enhance care delivery.

Experience/Qualifications

• Licensed Physical Therapist with current state licensure in good standing

• Provide comprehensive PT assessments and develop individualized plans

• Deliver evidence-based manual and modality treatments with measurable outcomes

• Achieve and document productivity targets and care pipeline visibility

• Strong documentation skills in electronic health records (EHR)

• On-site patient care with a results-driven mindset and outcomes focus

• Collaborate with multidisciplinary team to coordinate patient care

• Educate patients and caregivers on exercises, safety, and home programs

• Commitment to ongoing professional development and applying new skills

• Identify process-improvement opportunities to enhance care delivery
