Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.Community Healthcare of Texas is currently recruiting for a Field LVN.$2,500 Sign-on bonusPOSITION SUMMARYAssist the RN Case Manager in the plan of care for the patient.ESSENTIAL FUNCTIONSAssists RN in case management of total plan of care of patient/support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures, and regulations that define the scope of practice.Complete, maintain, and submit accurate and relevant clinical notes regarding the patient's condition and care given.
