Community Care Prescription Support

Major duties include, but are not limited to the following:

• Community Care Prescription Support:
  • Receives faxed, mailed, electronically prescribed, or delivered prescriptions written by Community Care providers and documents all received prescriptions in Electronic Health Record (EHR).
  • Reviews EHR to determine authorization to receive Community Care.
  • Contacts patients regarding prescriptions from unauthorized provider visits and routes or returns prescriptions appropriately.
  • Reviews authorized prescriptions for appropriateness using pharmacy technician knowledge, as well as knowledge concerning the national drug formulary and local VA formulary requirements.
  • Obtains prescription clarifications from providers and forwards updates to the reviewing pharmacist.
  • Initiates the non-formulary/prior authorization process with the provider and records needed documentation in EMR and workflow file.
  • Follows-up via phone and/or fax with Provider regarding initiated communications concerning non-formulary/prior authorization medications, clarifications, or medication change requests.
  • Inputs authorized prescriptions or prepare electronically prescribed medications in EHR for the review and verification by the Clinical Pharmacist or representative.
  • Contacts patients with updates on prescriptions and documents in EHR.
  • Assists in the tracking and shipping of prescribed controlled substances, as well as all prescribed medications mailed to patients.
  • Reviews and processes electronic cancellations of prescribed medications.
  • Communicates with CBOC representatives when authorization of prescription is requested or when prescription from courier has not been received.
  • Communicate with Procurement Pharmacist or Procurement Technician when specialty medications need to be ordered. Works with procurement team to notify community care providers and patients when medications and/or supplies ordered are unavailable to develop plans for alternative care options.
  • Utilizes pharmacy information systems to answer patient or provider questions related to status of their prescription(s), completion date, package tracking, etc.
  • Educates patients on how to use the automated phone service.
  • Transfers calls to the pharmacist, when needed and appropriate.
  • Accurately documents all relevant communications, patient requests, and clinical activities in the Electronic Health Record (EHR) or CPRS, ensuring timely alerts to providers as needed.
  • Functions as a resource to members of the pharmacy, medical center, and CBOCs regarding Community Care Prescriptions.
• Program Management: The Pharmacy Technician Performs a wide variety of administrative, management, and coordinating duties in support of pharmacy programs
  • Resolves ePharmacy rejects and processes them to be dispensed.
  • Sends rejects to Clinical Pharmacist for resolution when appropriate.
  • Interacts with patients, providers, other VA staff, and third-party payers as needed to resolve rejects.
  • Ensure rejects are resolved so that patient receives appropriate medications in a timely manner.
• Pharmacy Dispensing/Distribution Services: The Pharmacy Technician will be involved in assuring appropriate, safe, and accurate medications are dispensed to veterans in an efficient manner.
• Pharmaceutical Care: The pharmacy technician shall provide and support clinical and pharmacoeconomic activities as assigned. All assigned clinical activities and functions shall be completed in accordance with approved clinical pharmacy protocols.
• Patient Care Services: The pharmacy technician will promote a patient-focused, team-based multidisciplinary model of patient care.

Work Schedule: Monday-Friday 8:00am-4:30pm

Pay: Competitive salary and regular salary increases.
