What That Means for You Competitive pay: $48.00-$55.00 per hour based on experience, Routine Visit Rate: $60.00 Flexible scheduling that supports work-life balance A responsive clinical and leadership team that provides backup when you need it Professional autonomy to use your judgment and manage care in the home Meaningful one-on-one care with time to educate patients and families Benefits including mileage reimbursement and incentive programs What You'll Do Perform patient assessments and contribute to care planning Develop and update individualized plans of care aligned with physician orders and patient goals Deliver skilled nursing services in the home setting Educate patients and caregivers on conditions, treatments, medications, and self-care Provide preventive, restorative, and rehabilitative nursing interventions Monitor progress, identify changes in condition, and coordinate appropriate follow-up Maintain timely, accurate clinical documentation Collaborate with the interdisciplinary team to support continuity of care What You Bring Graduate of an accredited nursing program with an active Arizona LPN license Minimum of 1 year of LPN experience in Home Health preferred Home health, wound care, or case management experience preferred Current CPR certification Valid driver's license, auto insurance, and reliable transportation Strong clinical judgment, communication, organization, and documentation skills Why Work for Interim HealthCare of Queen Creek & Gilbert?
