We are seeking a licensed Physical Therapist (PT) to join our team and provide high-quality, patient-centered therapy services to residents in a supportive long-term care environment.

This is an excellent opportunity for a therapist who is passionate about helping patients improve their mobility, function, independence, and overall quality of life while working collaboratively with an interdisciplinary care team.

Key Responsibilities

Evaluate residents and develop individualized treatment plans based on clinical needs and physician orders.
Provide skilled physical therapy services in accordance with established care plans.
Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
Collaborate with nursing, therapy, and interdisciplinary team members to support resident outcomes.
Participate in care planning, utilization review, and interdisciplinary meetings as needed.
Educate residents, families, and staff regarding therapy goals, treatment plans, safety, and functional mobility.
Maintain established productivity, documentation, and quality standards.
Support regulatory compliance and survey readiness.
Promote resident safety, dignity, independence, and quality of life.

Qualifications

Graduate of an accredited Physical Therapy program.
Current and active state license as a Physical Therapist.
Long-term care or skilled nursing experience preferred.
Strong clinical documentation and communication skills.
Ability to work effectively as part of a multidisciplinary team.
Patient-focused, compassionate, and committed to providing quality care.

Physical Requirements

Ability to stand, walk, bend, kneel, and assist residents during therapy activities.
Ability to safely lift, transfer, and support residents as required.
Ability to work in a clinical and long-term care environment.
