Wellstar Health System is seeking a Fulltime RN Case Manager for Home Health in the Kennestone/Cobb area. You will provide and supervise primary care to 25-35 homebound patients, coordinating care across RNs, LPNs, therapists, social workers and physicians. The role emphasizes compassion, communication with patients and families, and adherence to CMS/Joint Commission guidelines.
