About the Role:

As a Physical Therapist (PT) at Avista Nursing and Rehab, you'll be responsible for designing and implementing individualized therapy plans to help patients achieve their maximum potential for independence and well-being. You'll work closely with our interdisciplinary team to ensure that each patient receives comprehensive care that addresses their unique needs and goals.

Responsibilities:

• Conduct thorough patient evaluations to identify physical therapy needs and develop personalized treatment plans

• Deliver high-quality physical therapy interventions, including exercises, modalities, and other treatments

• Collaborate with other healthcare professionals to ensure seamless care coordination and communication

• Monitor patient progress, adjust treatment plans as needed, and document patient outcomes

• Educate patients and their families on physical therapy techniques, home exercise programs, and other relevant information

• Participate in quality improvement initiatives to continuously improve patient care and outcomes
