RN Care Transition Educator - Home & Virtual Care
Option Care Home Health LLC
Option Care Health LLC is seeking an experienced RN to educate patients, caregivers, and facility staff on home-based care options and the OCH care delivery model. You will conduct patient assessments, coordinate with the sales team, and ensure timely hospital-to-home transitions with proper documentation and confidentiality. Local travel to partner facilities and hospitals is required; strong relationship-building and proficiency with MS Office will support daily care coordination and education. #J-18808-Ljbffr Option Care Home Health LLC
