Job Description Summary The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of the Option Care, pharmacy practice professional standards, and applicable regulatory entities. This entry‑level position is an introduction to clinical pharmacy practice in an alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Job Responsibilities Determine the suitability of individual patients for home care by collecting and organizing all patient‑specific information needed to make assessments in accordance with Option Care policies. Evaluate medical condition and prescribed medication therapy for home care suitability and prognosis with clearly defined outcome goals. Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for home care patients. When appropriate, make suitable recommendations for modifications in medication therapy or monitoring parameters. Collect and organize patient‑specific information to prevent, detect, and resolve medication‑related problems and provide appropriate medication therapy recommendations. Identify appropriate monitoring parameters for each patient and complete nutritional assessments. Modify pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient‑specific information. Demonstrate competence in pharmacokinetic principles, formulas, and assessment of patient laboratory values. Provide medication‑use education to Option Care staff, other professionals, home care patients and/or their caregivers using effective patient education techniques. Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings. Use a systematic procedure to communicate pertinent patient information. Prepare and dispense medications following Option Care policies, accurately calibrate equipment, and prepare medications to be appropriately concentrated, stable, and safely stored. Adhere to safety and quality assurance practices, prepare labeling in compliance with policies, and inspect final medication before dispensing. Comply with state/federal laws and USP standards, including DEA regulations and inventory controls for controlled substances. Manage the use, maintenance, and troubleshooting of medication administration equipment and related devices. Accurately program, maintain, and advise others on equipment use. Participate in assessing, managing, and reporting medication‑related incidents, equipment errors, and failures. Complete QAR documents per Option Care policy and monitor inventory discrepancies. Provide concise, applicable, and timely responses to drug information requests from health care providers and patients. Formulate responses based on literature analysis. Attend educational seminars and participate in CEU opportunities to maintain professional competency and meet state‑mandated CEU requirements. Manage the use of investigational drug products according to regulations and Option Care policies. Utilize pharmacy support personnel effectively, prioritize workload, and organize workflow considering resource availability. Ensure accuracy of work performed by support personnel. In all activities, comply with accreditation, legal, regulatory, and safety requirements. Maintain confidentiality of patient and proprietary information. Observe legal and ethical guidelines for safeguarding patient and Option Care proprietary data. Supervisory Responsibilities No supervisory responsibilities. Basic Education and/or Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license as granted by the State Board of Pharmacy. At least one year of experience in sterile IV admixture procedures, knowledge of non‑sterile compounding, immunizations, and medication therapy management, completion of an ASHP‑accredited Pharmacy Residency Program, completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or equivalent experience approved by the VP of Pharmacy or SVP of Clinical Services. Basic Qualifications Experience providing customer service to internal and external customers, meeting quality standards for services and evaluating customer satisfaction. Basic proficiency in Microsoft Word. Basic proficiency in Microsoft Excel. Basic proficiency in Microsoft PowerPoint. Travel Requirements Will travel up to 5 % of the time for business purposes (within state). Preferred Qualifications & Interests Completion of an ASHP‑accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Experience in home care, hospital, or other clean‑room setting. Management Responsibilities At least 1 year of experience in indirect management of team members, including assisting in the development, training and assignment of work/projects to other team members. Salary and Pay Range Salary will be determined by the applicant’s education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay Range: $102,221.94 – $170,369.91.
