Care Management Coordinator (RN)Remote | California

We're seeking an experienced Care Management Coordinator to lead and support a team focused on helping members access the care, resources, and services they need. This leadership role oversees care management programs, staff performance, and care coordination efforts while ensuring high-quality member outcomes.

• Supervise and mentor care management staff.
• Monitor team performance, productivity, and program goals.
• Support staff with member care plans and complex cases.
• Collaborate with providers, community organizations, and internal teams.
• Identify process improvements and support quality initiatives.
• Assist with member concerns and care coordination needs.
• Ensure compliance with HIPAA and healthcare regulations.

• Active, unrestricted California RN or LCSW license.
• Bachelor's degree required.
• 5+ years of clinical experience.
• 3+ years of leadership or supervisory experience.
• 2+ years of managed care experience.
• Strong communication, organization, and problem-solving skills.
• Valid California driver's license.

• Medi-Cal managed care experience.
• Master's degree.
• Knowledge of behavioral health and community resources.
• Bilingual in Spanish and English.

This is a Contract position based out of Santa Barbara, CA.

The pay range for this position is $50.00 - $55.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
